Service Incident - March 1, 2022 (Running Workflows)

Summary

We've identified a service incident in which some users are frequently seeing "Duplicate entry" failures coming from Cromwell when running workflows. 

Timeline

March 4, 2022 9:30 AM ET - Issue fully resolved - The Terra engineers completed the required maintenance in the platform that was causing workflow slowdowns. Platform usability should be back to normal.

March 1, 2022 10:50 AM ET - Issue mitigated - Terra engineers released a fix that should mitigate the issue. Initial results show that the error message should not occur anymore. Users may still experience long workflow runtimes.

March 1, 2022 8:15 AM ET - Issue noticed - A user first wrote in experiencing frequent "Duplicate entry" errors when running workflows that previously ran successfully. The error was determined to be coming from Cromwell. Terra engineers are looking into it.

Impact

Users may experience failures for some samples when running workflows. We are currently unsure if relaunching the failed samples would be successful. Users may also experience some slowness while the engineering team looks into the issue.
